We suggest the following wording:

General Bequest of a stated sum of money:

  "I give to The Oregon Humane Society, 1067 NE Columbia Blvd, Portland, OR 97211, Federal Tax Identification Number (Please contact us for OHS’s EIN/TIN number) ___________________Dollars ($___________)."

Specific Bequest of a certain asset from your estate:

Residuary Bequest, after other bequests and expenses have been paid:

Contingent Bequest if you are not survived by certain individuals:
